When it comes to eye aesthetics in facial feminization, two of the most important terms we encounter are “canthopexy” and “cantoplasty”. Although they are seen as similar procedures at first glance, there are actually big differences between them in terms of function and application.
For people who are considering an intervention to have almond eye aesthetics, to eliminate eyelid sagging or for a more dynamic look, knowing which of these two techniques is performed and when is very important for the best results around the eyes.

What is Canthopexy and In Which Situations Is It Preferred?
First of all, canthopexy is a procedure that involves surgically suspending the outer corner of the eye (lateral canthus) upwards. It creates the almond eye effect in the eyes. During this procedure, the structures are gently pulled, stretched and positioned upwards. For this reason, canthopexy is often preferred as it is a more minimally invasive technique. Canthopexy provides a long-lasting effect, but there may be slight changes over time due to aging. Recovery usually starts within 5-7 days after canthopexy. Canthopexy may take a little longer.
Canthopexy is generally preferred for the following cases:
- Pulling the eye shape upwards (almond eye effect),
- To prevent eyelid sagging,
- It is preferred to support and improve lower eyelid aesthetics.
In short, canthopexy aims to preserve the natural structure of the eye while visibly changing the eye shape.
What is Canthoplasty and In Which Situations Is It Preferred?
Cantoplasty is a slightly deeper and more comprehensive procedure than canthopexy. In this procedure, the outer corner of the eyelid (lateral canthus) must be completely cut. After the incision, the eyelids are reshaped and fixed at a higher point. This results in a more pronounced change in the eyelid.
Canthoplasty is preferred for the following conditions:
- Problems such as advanced eyelid laxity and drooping,
- For more lasting, defined and dramatic results,
- It is preferred in cases such as correction of eyelid deformities (for example, age-related entropion or ectropion).
In short, canthoplasty is an effective solution for structural problems or more serious aesthetic needs.
Canthopexy or Canthoplasty? Which Should You Prefer?
At this point, Dr. MFO will determine the best option after comprehensive evaluations. But in general:
Status | Appropriate Technique |
If you only want a slight slant | Canthopexy |
For combined procedures with lower eyelid surgery | Canthopexy |
Advanced eyelid sagging | Canthoplasty |
If you are aiming for a more pronounced, permanent and overall structural change | Canthoplasty |
Conclusion

Both procedures are effective procedures aimed at improving the eye contour. However, to address the main differences, canthopexy is a great option for those who want naturalness and minimal intervention, while canthoplasty is ideal for those who want more radical and permanent change. Here, preferences appear as the most decisive element of the procedure. In short, one can be considered as a “fine touch” and the other as a “new shape”.
Like every aesthetic intervention, eye aesthetic procedures should be planned individually and personal preferences should be well understood. The most suitable technique for eye shape, skin structure, age and expectations is determined by Dr. MFO and his team and performed in the most precise way.
