1. Glabellar Prominence (Brow Ridge)

Located right in the center of the face, between the eyebrows, this small bony protrusion is prominent in masculine faces and flatter in feminine faces. This protrusion, called “glabellar prominence,” is one of the points that most affects facial expression, yet goes unnoticed by many patients.
During feminization, the surgeon refines this area to soften the transition between the forehead and nose, filing down the brow prominence to give the face a gentler, calmer expression. The work done on this area is the unsung hero of forehead feminization surgery.

2. Radix (Nasal Root Point)

The radix is the point where the nose meets the forehead. In feminization, not only the tip of the nose but also this starting point is extremely important. 5. Alar Base (Nasal Wing Base)

Many patients undergoing feminization do not understand why the width of the wings is important when the nose is reduced. The term “alar base” refers to the base area where the nasal wings begin on the face. If this area is too wide, no matter how feminine the tip of the nose is made, the face will still appear masculine. With the alar base reduction procedure:

  • The nostrils are slightly brought closer together,
  • The nasal base (the triangular area under the nose) becomes more delicate,
  • The smile line and lip-to-nose ratio achieve a more feminine balance.

Although this intervention may seem minor, it is an important touch that determines the symmetry and elegance of the face, especially when viewed from the front.
