As time passes, the most common sign that appears on the face is wrinkles between the eyebrows. These wrinkles can negatively affect not only a youthful appearance but also a feminine appearance. Women who want to look young, as well as trans women, seek ways to get rid of these wrinkles because they negatively affect a feminine appearance.
Therefore, frown lines (glabellar lines) between the eyebrows are a frequently addressed area in facial feminization to achieve a softer, calmer, and more feminine expression. Solutions are evaluated as non-surgical and surgical options; they are often combined. Over-the-counter creams and minor skincare changes, especially when lines begin to deepen, generally have limited effect. However, for those seeking permanent results, surgical and various injection-focused procedures are more frequently preferred. In this article, we will address these issues with a focus on facial feminization.

Details of an Aging Face: Understanding the Causes
An aging face typically involves a gradual decrease in the production of collagen and elastin, two fundamental protein fibers that help maintain skin tone and firmness. Over time, the production of these proteins slows down, causing the skin structure to weaken.
Combined with a reduction in fat deposits in the deep skin layers, this process can lead to the formation of wrinkles that tend to deepen over time. While collagen loss is a natural part of aging and cannot be completely prevented, certain treatments can help increase collagen levels and provide a more youthful appearance. Furthermore, facial expressions, especially frowning, cause wrinkles in this area. At this point, surgical and non-surgical solutions help people get rid of this appearance. Which methods are preferred depends on the individual’s expectations.
Why is the Glabella Area Important?
The glabella is the area between the eyebrows where the mimic muscles work intensely. In male facial anatomy, this area is generally:
- More prominent,
- More bony,
- The distance between the eyebrows is narrower,
- The lines between the eyebrows tend to be deeper.
This structure unconsciously gives the facial expression a harsh, tense, and dominant appearance. The goal of facial feminization is to soften this harsh expression and achieve a more open, balanced, and feminine facial architecture.
Why Are Glabella Wrinkles More Pronounced in Facial Feminization?
Frown lines are not solely related to aging. They may be more pronounced, especially in trans women and individuals undergoing feminization, due to the following reasons:
- Prominent forehead bone structure
- Naturally lower eyebrow placement
- Strong corrugator and procerus muscles
- Years of stress and habitual facial expressions
When these factors come together, frown lines can form earlier and more deeply.
Non-Surgical Treatment Methods

Botulinum Toxin (Botox)
Botox helps reduce frown lines by relaxing the muscles responsible for wrinkles. This is the most common and first-choice method. This neurotoxin temporarily blocks nerve signals, softening the muscle contractions that cause wrinkles during facial expressions.
- Relaxes the muscles between the eyebrows (corrugator & procerus)
- Reduces stern/angry expression
- Eyebrows lift slightly → more feminine appearance
- Duration of effect: 3–6 months
In terms of feminization: With the correct dosage and technique, a soft expression is achieved without creating drooping eyebrows.
Filler Applications (Hyaluronic Acid)
Dermal fillers are another common method that uses compounds such as calcium hydroxylapatite, hyaluronic acid, and collagen to restore volume and improve facial contours. These substances can correct mild to moderate skin sagging and reduce the appearance of hollow areas. Fillers are injected directly into areas that have lost volume to help smooth lines and wrinkles.
- Preferred for deep and static wrinkles
- Gives better results when performed with Botox
- Duration of effect: 9–18 months
Caution: Must be performed by a highly experienced physician as the glabella region is risky in terms of blood vessels.
PRP / Mesotherapy
- Improves skin quality
- Effective on fine lines
- Does not eliminate deep muscle-related wrinkles on its own
- Is more of a supportive method
Laser / RF / Ultrasound (HIFU)
- Provides skin tightening and collagen increase
- Effective on fine lines
- Used in the feminization process for a “smoother and more vibrant skin”
Brow-Focused Facial Feminization Surgery

Facial feminization is a comprehensive process that aims not only to soften prominent bone structures but also to give the facial expression a more peaceful, open, and feminine appearance. One area that is often overlooked in this process but directly affects facial expression is the brow region, particularly the glabella (brow area).
Glabellar wrinkles can make the face appear harsher, more tired, or even masculine. Therefore, glabella-focused procedures play an important role in facial feminization surgery, both aesthetically and psychologically.
Forehead Contouring
There is often a bony protrusion beneath glabellar wrinkles. In this case, superficial interventions alone are not sufficient. With forehead contouring surgery:
- The bony protrusion in the glabella region is shaved down,
- The forehead-brow transition is made smoother,
- Deep shadowing between the eyebrows is reduced.
This procedure naturally softens the appearance of frown lines.
Brow Lift
Low-set eyebrows cause the lines between the eyebrows to appear deeper. With a brow lift:
- The eyebrows are raised to a more feminine angle,
- Tension between the eyebrows is reduced,
- The face takes on a more rested and open expression.
Endoscopic brow lift is one of the most frequently preferred methods in facial feminization.
Surgical Interventions Targeting Mimic Muscles
The main cause of wrinkles between the eyebrows is often the excessive activity of the corrugator supercilii and procerus muscles. During facial feminization:
- These muscles can be weakened or partially released,
- Preventing the reappearance of deep lines between the eyebrows,
- Achieving a more permanent and natural result.
This approach offers a surgical alternative to the temporary effects of Botox.
Soft Tissue and Skin Support Procedures
The following procedures may also be added to the brow area during surgical feminization:
- Fat injection (lipofilling)
- Deep filler supports
- Subcutaneous connective tissue reinforcement
These methods complement the results of surgical procedures and ensure a smoother facial expression.
How Do Brow Furrows Affect Feminine Expression?
When the lines between the eyebrows are reduced, the face gains a calmer, more feminine, softe and more approachable expression. Many people report that the “constantly angry or tired” look disappears after surgery. This change has not only aesthetic but also positive social and psychological effects.
The area between the eyebrows is one of the facial expression centers. Therefore, it is crucial that the procedures are planned to be:
- Subtle,
- Proportionate to the face,
- Without erasing the person’s character.
The goal is not a frozen face, but a natural, balanced, and feminine expression.
Small Area, Big Impact
When it comes to facial feminization, the nose, chin, or cheekbones are usually emphasized. However, the brow area is one of the areas that transforms facial expression most quickly and powerfully.
Properly planned surgical procedures focused on the brow area enhance the overall feminine perception of the face, soften the expression, and help the person feel more at peace with their reflection in the mirror.
